I've done many inserts using MediaFood software from here:

This was on their own media that came in perforated standard sheet sizes. The UK has the A4 size but it really depends on what you are printing on as to whether you'll spend much time aligning these templates to your media.

It seems to be readily adaptable but doesn't look quite the part these days. It is simple and works, IMHO.
